RESUMO

The aim of this dissertation is to cooperate in the elaboration of public policies, discussing implantation strategies for Worker s Health Programs (WHP), especially regarding the surveillance and prevention of work accidents, as well as contributing to the discussion of methods for investigation of causes and prevention of work accidents. Other specific objectives are to reconstruct the structure of the Worker s Health Program of Piracicaba, trying, through its history (1997-2002), to identify the central elements that allowed the construction of a privileged inter-institutional space for effective prevention. The qualitative approach is used by means of a documental research among the materials produced by Piracicaba s WHP within the period of 1997 and 2002; four cases of fatal work accidents which happened in the years of 1998, 1999 and 2000 and their preventive outcomes are studied; the comparative study of two cases investigated by the WHP as well as the Labor Ministry of Piracicaba and the Criminal Studies Institute was done, verifying the different approaches used in the reports. Finally, a documental research and a descriptive analytical study was done in 71 reports from the Criminal Studies Institute of Piracicaba :from the years of 1997, 1998, 1999 and 2000. Four central elements are identified in the inter-institutional construction of Preventive Measures in the city of Piracicaba, which are grouped as needs, demands, information systems, and actions priorities. In the investigation of four fatal accidents in the civil construction and paper manufacture areas by the Causes Tree method, we found a lack of basic safety measures for machines and equipment as well as load elevators in civil construction. Afier the application of interdiction and firne penalties, the cases originate processes of sectorial negotiations for the prevention of more accidents in each respective area. A comparison of the reports :from the accidents investigation is done both by the Worker s Health Program and the Criminal Studies Institute in order to evaluate the results and compare both approaches. The research shows the importance of inter-institutional actions, the partnership among the Worker s Health Program and the Municipal Health Secretary of Piracicaba and members of the Labor Ministry, the integration of the service with local universities and representatives ofthe civil society in the prevention and promotion of health. Finally, a system is proposed for the surveillance against severe and fatal accidents in form of sentinel events, which includes a information system with several sources, the active search of cases (rescue systems, police stations, emergency rooms) and the reporting of incidents in the emergency rooms, resulting in preventive actions and collective and sectorial negotiations for the prevention ofthe most relevant risks. The study also shows that the Criminal Studies lnstitute uses partial methods and conceptions in its investigations, which are based on a theoretical model (the victim s insecure action) which puts the worker as the sole responsible for the events, favoring employers instead of a prevention policy and a more accurate investigation of the causes and responsibilities resulting ITom these events
