Condição periodontal de crianças e adolescentes com diabetes mellitus tipo I

RESUMO

In a transversal study, we evaluated the periodontal condition of 168 children and adolescents with DM type 1 and its relation with their metabolic control. Patients were followed-up at the pediatric endocrinology division of Hospital das Clinicas in Belo Horizonte, Brazil. The sample of both sex (44% male), aged 7 to 19 years old, non smoking, was analised in two groups: individuals with mix dentition and with permanent dentition. DM1 duration varied from 0.4 to 18 years. In the clinical exam it was evaluated the plaque index (PI), sites with bleeding on probing (BOP), probing depth (PD . 4mm) and clinical attachment level (CAL . 3 mm) in all oclusion permanent teeth, with the exception of the third molars. The IPL and BOP exams were also performed in deciduous teeth. Glicemic control was assessed through Hemoglobin A1c (HbA1c) levels of the 2 last years. It was observed a prevalence of 20.8% of gingivitis and 5.9% of periodontitis in this population. The percentage of individuals with BOP were significantly higher among those with abundant plaque (P<0,001). The individuals with poor metabolic control had higher percentage of affected sites on PD (p=0,004) and on CAL (p=0,014). It was found significant associations among alteration in periodontal parameters and DM1 duration: individuals with more than 5 years of DM1 showed higher percentual of affected sites on PD (p=0,002), on BOP (p<0,001) and on CAL (p=0,007). The results showed that DM1 duration and poor glycemic control were associated with worse periodontal clinic parameters, indicating higher susceptibility in developing the DP.
