Comparação dos níveis de sedação graduados pela Escala Comfort-B e pelo Índice Biespectral de crianças em ventilação mecânica na UTI pediátrica

RESUMO

Introduction: The use of sedative and analgesic therapy, which is frequent at ICUs, makes indispensable the constant re-evaluations of the patients¿ level of sedation. Several instruments to measure sedation have been used, from clinical scales to monitoring of the level of consciousness. Objective: To compare sedation scores resulting from the COMFORT-B scale, applied by independent judges, to the Bispectral Index (BIS) in children staying at the pediatric ICU at Hospital de Clinicas de Porto Alegre, who were subjected to mechanical ventilation and use of sedative medication. Patients and methods: One-month to 16-year-old children, divided into two groups (with and without neurological injury), who were subjected to mechanical ventilation and use of sedatives, were assessed. They were monitored and ranked by both BIS and Comfort-B scale, at the same moment, and their behaviors were digitally recorded. Later, the film was used by other three independent observers to rank the patients¿ sedation. The results obtained by these judges were compared, and the accordance among them was tested (Bland-Altman and Kappa tests). The correlation of data obtained through both monitoring and application of the scale was also tested (Pearson correlation test). The research was approved by the Ethics and Research Committee of the institution, and there were no conflicts involved in the investigation. Results: Seventy observations were performed in 19 patients (eight of them presenting with neurological injury). The scale ranking showed high agreement level among judges (minimum of 94.3 percent and maximum of 97.1 percent), with Kappa values between 0.542 and 0.683, and p<0.001. There was a positive, regular, statistically significant association between BIS and Comfort-B, both in the group with neurological damage (r = 0.720 to 0.914 and p<0.001) and in the group without neurological damage (r = 0.424 to 0.498 and p = 0.002 a 0.011). Conclusion: The agreement level between the independent judges was high, and the correlation between the Comfort-B scale and BIS was strong in the group with neurological injury and regular in the group without neurological injury. Hence, it is possible to conclude that Comfort-B scale is reproducible and useful in ranking the level of sedation in children undergoing mechanical ventilation.
