Caracterização e ritmos biologicos de parametros bioquimicos sericos em uma população brasileira

RESUMO

Laboratory exams are subject to several interfering aspects and even healthy individuaIs may present variations, stemming from biological and/or laboratory factors, and generating problems in the outcome interpretation of exams. As a consequence of laboratory technological improvement, information technology and strict quality control systems, a marked decrease in analytical and post-analytical variability has taken place in clinical laboratories today. Accordingly, studies have preferably focused on the origin of pre-analytical errors. Among the biological components current1y being investigated there is the intra-individual, which is inherent to the individual and changes over time. The objective of this work was to evaluate the occurrence of biological rhythms in several biochemical serum parameters (correlating with atheroscleroses) and to measure the prevalence of dyslipidemia, as well as the modulation of serum lipoprotein(a), in a Brazilian population. For such four retrospective studies and a prospective one the results of severaI biochemical serum parameters of individuaIs registered at a referral University Hospital in Campinas - Unicamp,duringthe yearsof 1996to 2003 were evaluated. The analyzed parameters were: uric acid, calcium, cholesterol, creatinine, phosphorus, HDL-cholesterol, LDL-cholesterol, lipoprotein(a), triglycerides and urea. Statistical analyses were carried out by the SAS or SPSS programs for descriptive and comparative analyses. Temporal analyses were carried out using the Cosinor method. The values found for cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides and Lp(a) are out of the recommended ranges by NCEP and adopted by IIIDBSD in a great portion of the studied population. Seasonal rhythms was observed in serum cholesterol, HDL-cholesterol, LDL-cholesterol, calcium ~nd creatinine. The correlation between the rhythms of cholesterol and LDLcholesterol and the prevalence of manifestations of atherosclerosis strongly suggest their impact of seasonality on coronary artery disease. In conc1usion, the serum concentrations of cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides and Lp(a), so much in aduIts as children and adolescents, indicated the high prevalence of dyslipidemia in this area. The sazonality observed for cholesterol, HDLcholesterol, LDL-cholesterol, calcium and creatinine should be considered a significant cause of pre-analytical variation in these laboratory tests and also the rhythms of lipids and lipoproteins are associated with atherosc1erosis rhythmicity in the studied area. These results emphasize the importance of "biological c1ocks" in medical practice
