CARACTERÍSTICAS CLÍNICAS, EPIDEMIOLÓGICAS E CONCENTRAÇÕES DE SÓDIO E POTÁSSIO NO COLOSTRO DE NUTRIZES HIPERTENSAS E NÃO HIPERTENSAS, ATENDIDAS EM UM SERVIÇO DE SAÚDE EM MARIANA, MG

RESUMO

Colostrum, the first milk secreted after delivery, is rich in proteins, vitamins and minerals, sodium and potassium amongst them. These are essential nutrients to the organism and are involved in the regulation of osmolarity, pH and volume of corporals fluids. The role of sodium in the regulation of volemy is well documented and a high sodium intake, especially by sal-sensivite individuals, is considered as fundamental in the genesis of arterial systemic hypertension. Potassium exerts a protective function against hypertension in the presence of a high sodium intake. The aim of this study is comparing the concentrations of sodium and potassium in the 48-hour colostrum of 105 breast-feeders mothers in postpartum period, 33 hypertensive and 78 non hypertensive, attended at SBSC - Monsenhor Horta hospital, Mariana, MG, from june to november 2003. Data from these mothers were obtained from a structured interview and from a search in the medical registers of patients. Colostrum collection followed the technique recommended by Vinha (1987, 1994) and ion determinations in colostrum were done according to Keenan et al. (1982). Mothers were selected for the groups of non hypertensive (68,8%) with average of admission blood pressure of 111/70 mmHg and hypertensive (31,4%) with average of admission blood pressure of 159/103 mmHg. The average concentrations of sodium and potassium obtained from 48-hour colostrum of non hypertensive mothers were respectively 87,2 ( 14,31) mmol/L and 14,45 ( 4,24) mmol/L; amongst the hypertensive ones the values were 87,75 ( 11,83) mmol/L and 14,40 ( 4,24) mmol/L. When associations of hypertension with colostrum levels of sodium and potassium, as well as the variables: age, parity, family (father and mother) history of hypertension, gestational age, weight of the newborn and socioeconomical factors were tested, only age and parity were significantly associated with hypertension (p≤0,05). When associations of sodium and potassium concentrations with the above cited variables were tested, positive results (p≤0,05) were found between age and colostrum potassium concentrations. In the present study the average concentrations of sodium and potassium in the colostrum of non hypertensive and hypertensive breast-feeders mothers showed no significant differences.
