Avaliação microbiológica, clínica e da matriz metaloproteinase - 8 (MMP-8) na peri-implantite.

RESUMO

The aim of this study was to evaluate the microbiological, clinical and immunological parameters of peri-implantitis. A total of 44 subjects (mean age 48.9+13.51 years) that present implant restored and in function for at least 1 year were divided in 2 groups: peri-implantitis (n=22) characterized as peri-implant sites with radiographical defects with a saucer shape >3mm, and bleending on probing; and control group (n=22) subjects with health implants. Clinical assessment including dichotomous measures of gingival redness, plaque index, bleending on probing and suppuration as well as calibrated measurements of probing depth and, clinical attachment level were made at 6 sites per implant. Supra- and subgengival samples were taken separately and analysed for 38 microorganisms by Checkerboard DNA-DNA Hybridization. The level of matrix metalloproteinase 8 (MMP-8) in peri-implant crevicular fluid were also evaluated. All clinical parameteres were statistically higher in diseased implants, with exception of plaque index. The total count of microorganism of red complex as Porphyromonas gingivalis, Treponema denticola, and Tanerella forsythensis were higher in the peri-implantitis for both supra- and subgengival biofilms (p<0,001). All the microorganism detected in subgengival biofilm were present in supragengival biofilm. The presence or absent of P. gingivalis and P. intermedia in supragingival biofilm presented higher correlation with the presence or absent of the same species in subgengival biofilm. MMP-8 presented higher levels at diseased sites (p<0,0001) as well as strongest correlation with microorganisms of red complex. The data of the present study showed a significant difference between supra- and sungengival biofilm of health and diseased implants. The total counts of some microorganism of red and orange complexes as well as the levels of MMP-8 were higher in supra- and subgengival biofilms of diseased implants.
