Avaliação clinica e hematologica em crianças filhas de mães infectadas pelo HIV-1

RESUMO

Hematologic alterations are frequent in AIDS, anemia being the most commonly reported. In this study we analysed 79 children , whose mothers were infected with HIV -1. Of these, 26 were infected perinatally and 53 were exposed but uninfected children (seroreverters). The clinical follow-up of these children was carried out at the Immunodeficiency division at the Clinical Hospital - State University of Campinas, Unicamp. The aim of the study was to analyse clinical and laboratory aspects of these children, especially complete blood counts, and to compare both groups. We intended to verify: the percentage of children with early clinical manifestations under one year of age; the association between infection with HIV -1 and the use of AZT during pregnancy; breast feeding; serum ferritin levels; the presence of anemia and lymphopenia. The results showed failure to thrive in both groups, a little more accentuated among the seroreverters. Also, in both groups there was an inadequate quality of life. The human immunodeficiency virus pediatric classification system, clinical category, showed: 4 children in category N; 7 in category A and 15 in category B. Clinical manifestations under 12 months of age were seen in 18 infected children (69.2%). They developed preferentially lymphoproliferative syndrome. Uninfected children had preferentially anemia. Among the children of the 15 mothers who took AZT during pregnancy, three developed HIV-1 infection. Breast feeding was observed in 16 infected children (61.5%) and in 8 (15. 1 %) of the seroreverters. The hematological alterations predominated among the infected children, mainly anemia, with significant statistical difference relation to the seroreverters. The etiology of anemia in the infected children was the same, as that of a chronic disease while in the uninfected it was iron deficiency. A statistically significant association between ferritin levels and HIV-1 infection or of the immunologic and clinical classification with anemia and lymphopenia was not observed
