Avaliação clinica de quatro metodos de diagnostico de lesões de carie oclusal

RESUMO
The goaI of this study was to compare different clinicaI diagnostic methods: fluorescent laser (DIAGNOdent system), direct digital radiograph (CDR system), bitewing radiographs and visual inspection for occlusal surf.aces. The sample w.as constitutM of 153 first permanent molars with microcavities of 6 to 8-year-old children ofboth sexes, who live in Piracicaba, SP. The validation method used was operative intervention. The sensitivity and positive prediction values were calculated to diagnosys methods. The results showed that by visual inspection 96 teeth had microcavity lesions and of these, 57.3% were in enamel and 42.7% were in dentine. In 57 teeth, an association was found between white spite lesions and microcavities, of which 91.2% were in dentine and on1y 8.8% were in enamel. The values concerning the sensitivity of the methods on enamel lesions were 0.99 for fluorescent laser; 0.77 for bitewing radiographs; and 0.75 for direct digital radiograph. The positive prediction values were 0.97 for fluorescent laser; 1.00 for bitewing radiographs; and 0.77 for direct digital radiograph. The sensitivity values of the methods on dentinal lesions were 0.99 for fluorescence laser; 1.00 for bitewing radiographs and 0.95 for direct digital radiograph. The positive prediction values were 0.80 for fluorescent laser; 0.92 for bitewing radiographs; and 0.94 for direct digital radiograph. The fluorescent laser showed 19.3% false positive decisions. In conclusion, microcavity lesions associated with white spot lesions can be considered a good criterion for diagnosing dentinal lesions. The accuracy of the radiograph method was low for detecting enamel lesions and high for detecting dentinal lesions. Fluorescent laser can be considered a good method for diagnosing initial lesions, if taking into consideration the high percentage of false positive decisions
