Avaliação clinica da utilização do enxerto de matriz dermica acelular (AlloDerm) em retrações gengivais

RESUMO

The aim of this study was to clinically evaluate the coverage of gingival recessions treated by an acellular dermal matrix allograft" (ADMA - test group), used as subepithelial connective tissue graft and compared it to the coronally repositioned flap (CRF - control group). Paired gingival recessions of ~ 3,Omm, Miller s class I, in upper canines or premolars teeth, were selected in 13 patients. Each patient showed two contralateral gingival defects, which were randomly assigned for the test group ar the control group. Clinical measurements were taken immediately before the surgery and at 6 months of postoperative. Clínical parameters included were: probing depth (PD), clínica I attachment levei (CAL), recession height (RH), recession width (RW), height of keratinized tissue (HKT), thickness of keratinized tissue (TKT), plaque index (PI) and gingival index (GI). Intragroup (baseline x six months) and intergroup (test x control) were carried out. Mean baseline recession for the test group was 3,46mm and for the control group was 3,58mm. 80th treatments resulted in a significant root coverage (p = 0,000), reaching an average of 2,6mm in the test group and 2,5mm in the control group at the six month evaluation. The difference of recession reduction between the treatments was not statistically significant (p=1,000). The test group showed a mean percent root coverage of 76,2% and the control group 71,2%. There were no statistical significant differences between the treatments for PD, CAL, RH, RWand HKT however, the mean gain in TKT was 0,71 mm for the test group sites and for the control group sites was 0,24mm (p=O,OOO). It can be concluded that an ADMA and the CRF are able to promote efficient result in gingival recession treatment, however, a superior thickness of keratinized tissue can be expected after ADMA
