Associação entre a presença de trombofilias adquiridas e hereditárias e a ocorrência de abortamentos recorrentes, perdas fetais em gestantes de alto risco / Association between the presence of acquired and inherited thrombophilia and the occurrence of miscarriage, fetal loss in pregnant women at high risk

RESUMO

The presence of thrombophilia has been associated with most maternal venous thrombotic events and adverse complications of pregnancy. To check for association between the occurrence of miscarriage, fetal loss with the presence of hereditary and acquired thrombophilia study was performed involving 180 pregnant women divided into two groups, group AB - women with a history of recurrent abortion and fetal loss and group CO - women comparison, an Ambulatory of High Risk Pregnancy, School of Medicine Dr Hélio Mandeta Federal University of Mato Grosso do Sul (FAMED-UFMS). Were traced to the presence of acquired thrombophilia through research of anticardiolipin (aCL) IgM and IgG, lupus anticoagulant (LA) and anti-β2 glycoprotein I (anti-β2GPI) and the presence of hereditary thrombophilia through research deficiencies of protein C (PC), protein S (PS) and antithrombin (AT), hyperhomocysteinemia and factor V mutation (Leiden). Tests for thrombophilia screening of these were performed during the prenatal and were not repeated after the puerperium. The presence of inherited or acquired thrombophilia in isolation was found in 50.0% (50/100) of patients belonging to group AB and 31.3% (25/80) of pregnant women belonging to the CO group. The hereditary thrombophilias or, acquired simultaneously, were observed in 32.0% (32/100) of women in the group AB and 26.3% (21/80) of women in the CO group. Of the acquired thrombophilias more common among pregnant women in group AB and BC was the AL found in 20.0% (20/100) and 16.3% (13/80) respectively. In group AB the most common hereditary thrombophilia was PS deficiency found in 31.0% (31/100) of pregnant women while in group C met AT deficiency in 17.5% (14/80). It was concluded that there is a positive association (p = 0.0005) between the presence of hereditary and acquired thrombophilia and the occurrence of miscarriage and fetal loss.
