RESUMO

The leprosy reactions are inflamatory phenomena which occur during the evolution of the leprosy. Evaluating the laboratorial characteristics as well as the clinical evolution of the individuals with reactional episodes, mainly after the treatment it is necessary or the clarification of its contribution with the relapse. It is an analytic study control-case type, prospective and of observation character. the cases of leprosy reaction were both clinically and laboratorially compared after the treatment of leprosy with the multidrug therapy (MDT/MB) with the control group without the mention of reaction after discharge, paired by year/discharge of the treatment range of about 6 months, in order to analyze the possible association between the leprosy reaction after discharge, and the bacilli charge, using the anti PGL-1 (ML Flow) serum test. The study was held in two different reference hospitals in Recife-Pernambuco-Brazil where 208 multibacillary patients treated with (MDT/MB) having been correlated to the reaction after discharge with the bacilli charge through the anti PGL-1 (ML Flow) test and the baciloscopy, besides the variables: gender, age, clinical form, initial baciloscopy index (IBI), reaction during the treatment, therapeutic scheme, degree of incapacity and type of reaction. The association between the reaction after discharge and bacilli charge and the other variables investigated were checked by using the following: calculation of the odds ratio (OR), confidence break (IC 95%) and value of p. It was observed that the results found show that the reaction after discharge is statistically associated to the bacilli charge through the positivity of the serum after discharge (anti PGL-1) (OR 10.4) and with baciloscopy after discharge (OR 7.21), IBI >2.0 (OR 21.93), besides the association with the male gender (OR 2.07) age up to 60 years old (OR 0.26), with the virchowiana clinical form (OR 3.7) with the presence of leprosy reaction during the treatment (OR 4.33) and the reaction 2-type during the treatment (OR 4.34)a multivaried analysis was performed where it was observed that the magnitude of the "odds ratio" wasnÂt modified. By evaluating the estimated results as a whole that lead to the association between the reaction after discharge and the presence of bacilli charge through the positivity of the antibody anti PGL- 1, besides of clinical and laboratorial remarks for the occurrence of the reaction after discharge. It is possible to conclude that it is fundamental that the group and the health services are adequately prepared to conduct these episodes, and should be alert to the situations after the clinical cure. However, future studies will be necessary to determine when these individuals show risk of developing relapse
