Análise da influência do treinamento muscular respiratório ambulatorial no preparo pulmonar pré-operatório em indivíduos com afecções esofágicas.

RESUMO
Surgical procedures interferes the pulmonary functions leading the emergence of respiratory dysfunctions which are important causes of morbidity and mortality. Respiratory pathophysiological changes are observed in the postoperative period of individuals with or with no pulmonary diseases. These changes may be responsible for the occurrence of postoperative pulmonary complications and thus an increase of the hospitalization period. The aim of this study was to evaluate the effectiveness of outpatient pulmonary preparation in the preoperative period through a Muscular Respiratory Training (TMR) in individuals with benign diseases of the esophagus. To evaluate the effectiveness of pulmonary preparation were analyzed seventeen patients (12 males and 5 females) with disorders of the esophagus. Of these 17, fourteen patients had a diagnosis of advanced megaesophagus and three diagnosis of caustic stenosis. All patients received initial assessment that consisted of: Pulmonary function test (spirometry), evaluation of inspiratory (PImax) and expiratory (PEmax) muscle force, evaluation of airway obstruction through the Peak-flow and chest X-ray. After the evaluation, TMR was started up and performed at the clinic, with sessions of three sets of ten repetitions daily for a period of 4 weeks. Non-parametric test and T Students test was used in accordance to the distribution of the variables of Kolmogorov and Bartlett for the analysis of normality and homogeneity, respectively. The significance level was p = 0005. A significant decrease in the preoperative and postoperative hospitalization period was observed in the studied patients. Was verified a significant increase of the PImax, PEmax and Peak-flow values after the TMR, when compared to the initial values. The same did not happen in relation to the values of FVC, FEV1 and PEF. This study concluded that the TMR done outpatient was effective in increasing the strength of the inspiratory and expiratory muscles and reduced the hospitalization time.
