Adesão ao tratamento de pessoas com hipertensão arterial e complicações associadas: espaço para o cuidado clínico de enfermagem.

RESUMO

The research has as objective to evaluate the adhesion to the antihypertensive treatment in people with hypertension and associated complications followed in a center of reference in Fortaleza-Ceara. It is a descriptive study, with a quantitative nature, carried through with 79 people. It was used a form with questions about the social, demographic, clinical characteristics, use of treatment without medicines and pharmacological treatment for these people and elements in its therapeutic adhesion. The results had demonstrated predominance of the masculine sex (67.1%); age equal or superior to 60 years (64.1%); white color (77.2%) and 69.2% of retired or pensioners. About 43 (57.3%) had basic education; 67.7% of the people received <1|-|2 wages; 81% were catholic; 67.1% were formally married or lived in a consensual union and 48.1% had between 3 or 5 children. Of the total, 78.5% inhabited with 3 to 6 people and 86.1% lived in the capital. About the clinical characteristics, we have that 55.7% of people had related their time of diagnosis of the hypertension between 1|-|10 years; 27 (34.2%) had related to discover the illness for occasion of its complications. Amongst the complications found associated to the hypertension, it had predominance of the cerebral vascular accident (38); and Infarction of the myocardium (22). The majority (87.3%) already had been submitted to the hospital internments and had familiar history of hypertension (72.2%), diabetes (44.3%), acute cerebral vascular accident (35.4%) and infarction of the myocardium (30.4%). Only 19 people had presented controlled pressure ciphers. About the adhesion to the treatment without medicines, 53 (78%) people were overweight or with obesity and 43 (61.4%) presented the abdominal circumference increased. Of the total, 93.7% did not add salt to the meal. About 57% were sedentary and 51.3% judged themselves easily stressed. Amongst the 79 participants, 6.4% of the people had declared the current tobacco use and 17.7% had affirmed current alcoholic beverage use. The pharmacological treatment was indicated for all the people with arterial hypertension in the study, having been that in 40 people the current medicine taking was convergent with the medical prescription, even so 46 (58.2%) people already had interrupted the medicine taking at least a time. Difficulties in the pursuing of the treatment of the arterial hypertension had been identified related to the patient (19=24.1%), to the medication (28=35.4%), the treatment without medicines (25=31.6%) and institutional (35=44.3%). About 82,3% received aid from the family in the pursuing to the antihypertensive treatment. The analysis of the data still allowed to infer that the participants felt the desire to know more on its conditions of health, we just need to see some questionings presented. The study allowed to conclude that the adhesion to the treatment still does not represent a great challenge to the professionals of health and patients, marking with that the development of complications in people with arterial hypertension is a reality with negative repercussions in the life of these people and in all the health system.
