RESUMO

Background: Although the postoperative infection rate have decreased with prophylactic use of antibiotics, the inappropriateness of surgical antibiotic prophylaxis is a well documented worldwide problem. Methods: This study describes the process of implementation of a perioperative antibiotic prophylaxis protocol emphasizing the contribution of the pharmacist to the success of the program, and evaluates with a descriptive study design the impact of the protocol on the appropriateness of prophylaxis in private university hospital. For a period of one month before and one month after the implementation of a protocol all surgeries were evaluated for antibiotic prophylaxis. The appropriateness of the indication rose ITom 56.4% to 100.0%, and that of maintenance of postoperative PA, from 21.4% to 95.7%. The cost of the perioperative antibiotic prophylaxis/surgery fell 40.5%. Conclusion: Our paper shows how educative and managerial activities together can effectively promote changes in the surgical antibiotic prophylaxis in our institution. When in the post-protocol period a deviation from the protocol was identified in the daily pharmacy report the prescriber was individually contacted and a consensus decision was reached
