RESUMO

In this dissertation an analysis is made, from 1995 to 2002, of the service sector in the city of Recife, state of Pernambuco, Brazil. It focuses the health establishmentÂs rapid growth and diversity in Recife. Recent State Government data have shown that for every bedstead at hospital and related health establishments five jobs are created. Unlike other traditional economic sectors in the State, this ratio is expected to increase, it is contended, with technological innovation. The analysis of the health sector of Recife is carried out on the basis of data from direct interviews with managers of private hospitals and medical institution representatives and data collected from official sources. The analysis stresses the importance of economic structure, emphasizing the private sectorÂs role, employment creation, work force schooling and income, and the sector as a source of tax revenues. This dissertation has three different parts. In the first, a review is made of the relevant literature as a background to draw on the present situation of the service sector where health establishments are located. In the second, the analysis is based on the data available, basically: (i) employment data from RAIS-CAGED/MTE, collected by the Labor Ministry in Brazil; (ii) Health Statistics collected by IBGE â BrazilÂs Institute of Geography and Statistics; and (iii) tax, COFINS4[1], ISS5[2] and other government revenue from Receita Federal and Secretaria das FinanÃas do Recife. Also the data were used from DATASUS6[3] of The Ministry of Health. In the third, the information based on the interviews with health experts, hospital managers and medical institution representatives, was used. A number of conclusions were drawn from the study results. They help to explain the emergence of a diversified Health Service Cluster in Recife. First, and probably foremost, is the existence in Recife of a reliable higher educational infrastructure that provides training in various fields of medicine and health assistance. Second, is the recent process of the health service restructuring, in the wake of privatization, in the country with its influence in the private sector growth. Besides, one must not forget the effects in the economy and in economic activities like the health sector of the recent monetary stabilization. It is also argued that in spite of all the improvements that took place in the Health Sector in Recife, it cannot be characterized, theoretically speaking, as a complete economic cluster. Some stages have been attained in that direction but much has as yet to be done. On the other hand, it goes without saying this does not imply to accept that new stages to form a complete cluster cannot be attained
