Proposição de atributos e implementação de um módulo de banco de dados para prontuário eletrônico de pacientes vítimas de queimaduras

RESUMO

Increased morbidity and mortality and the lack of standardized procedures for collecting data on healthcare provided to burns victims require more enhanced studies to be performed. The aim of this thesis was to propose attributes for and the implementation of an electronic medical records module for recording information and analysing clinical, surgical and laboratory data relating to burns patients having inpatient hospital treatment. A multidisciplinary approach using medical and information technology knowledge was applied in attribute selection and database creation and testing. A selection of 210 attributes was made and studied obtained from specialist national and international medical literature and validated by means of a survey with 60 specialists involved in the treatment of these patients. At the end of the study 343 attributes were selected and grouped into 23 tables used to create the database. A free Database Manager System was used to create the database. Clinical, surgical, laboratory, demographic and statistical information was entered on the database, having been collected from patients admitted to medium and high complexity burns units at a university hospital accredited by the Brazilian Health System in a state capital in southern Brazil. 1832 lines of data input codes were generated, using the structured query language (SQL), in order to include the data on the database. During the applications and report generation creation phase it was possible to obtain information from multiple tables, which in turn enabled the comparison of multiple data. It was demonstrated that by standardizing data and information collection about thermal injuries improved knowledge can be obtained about the clinical condition of burns patients during their stay in hospital. Offering conditions for this knowledge to be scaled up requires an interrelationship between different areas of scientific knowledge in the adoption of strategies for the development of specific databases. In the short and medium term the creation of warning, decision-making support, diagnosis support, research and teaching systems is envisaged, as well as an information system on trauma, thus helping to minimize morbidity and mortality generated by these injuries, especially with regard to burns.
