Camada de fibras nervosas da retina em portadores de esquistossomose hepatoesplÃnica: anÃlise com laser confocal polarizado

RESUMO

It was evaluated the retinal nerve fiber layer thickness in patients with the hepatosplenic schistosomiasis mansonic with scanning laser polarimetry. It was performed a case-control study. The case group was represented by 50 eyes of 25 schistosomotic patients (15 males and 10 females), mean age of 51.6 +/- 9.6 years (range 37-73 years). The control group included 46 eyes of 23 non schistossomotic patients (14 males and 9 females), mean age 50.2 +/- 11.2 years (range 37-76 years). All eyes presented: best corrected visual acuity of 20/40 (0.5) or better; intraocular pressure of 18mmHg or less; cup disc ratio <0.4 and symmetry cup disc ratio between fellow eyes <0.2. It was used a GDxTM Glaucoma Scanning system (Laser Diagnostic technologies, Inc., San Diego, CA, USA). Two consecutives images were obtained after adjusting the focus, the brightness and the centralization of the optic disc for each eye and the average image was selected for measurement. The margin of the optic disc was approximated by a circular band located 1.75 disc diameters from the center of the optic disc around the inner margin of the peripapillary scleral ring. All parameters were used for the study. The values of average thickness (p = 0.01), ellipse average (p = 0.01), superior average (p <0.00), inferior average (p = 0.05) and superior integral (p = 0.05) were statistically significantly lower in the hepatosplenic form of schistosomiasis group than those in the control group. The others parameters those represents ratios (Symmetry,Superior ratio, Inferior ratio e Superior to nasal), modulations (Maximum modulation and Ellipse modulation) and the special parameter (The number) were not statistically significant. In the present study, the result suggests that case group had significantly damage to the retinal nerve fiber layer. The hepatosplenic schistosomiasis mansonic is responsible for lesions that can alter the systemic circulation. Therefore, hemodynamic changes of mansonic schistosomotic patients could affect the retinal nerve fiber layer
