Avaliação das notificações compulsórias de suspeita de maus-tratos contra crianças e adolescentes do município de São José do Rio Preto

Violence against children and adolescents is not a recent event, but a usual practice throughout history, justified and accepted by different societies. Currently, it is considered a great public health problem, affecting and impairing children and adolescents during an important period of development. The most frequent types of abuse are neglect or abandonment, physical, sexual and psychological abuses. The objectives of this study were to characterize types of abuse, victims, and aggressors and to identify the origin of notification. Method and Casuistic: This is a retrospective study of 137 cases of abuse suspect against children and adolescents. All Compulsory Notification Files sent to the Secretaria Municipal de Saúde e Higiene de São José do Rio Preto, São Paulo, from May 2002 to November 2003 were analyzed. Results: A total of 137 notifications were analyzed - 106 (77%) isolated violence cases and 31 (23%) associated violence (more than one type of abuse). Among the cases of associated violence, four (13%) included four types; eight (26%), three types and 19 (61%) two types; totalizing 185 occurrences of violence in the 137-analyzed notifications. The most frequent type of abuse was neglect (n:80; 43.5%), followed by physical (n:50; 27%), sexual (n:31; 17%) and psychological (n:23; 12.5%) abuses. Females were the most frequent victims (n:78; 57%); the age group most affected was between zero and nine years (n:84; 63.50%), and the mother was the main aggressor (n:53; 38.6%). The largest number of notifications was sent by the Unidade Básica de Saúde do Jaquaré (n: 58; 42,5%), followed by the ones sent by the Emergência de Pediatria do Hospital de Base (n:31; 22.6%). Conclusions: Neglect was the most frequent type of abuse, the victims were mainly female aged under nine years. The main aggressor was the mother, and the Unidade Básica de Saúde do Jaguaré sent the largest number of notifications. These data can help mapping domestic violence in Brazil, where there is lack of epidemiological studies on the problem. Further research must evaluate intervention programs aiming to reduce violence against children and adolescents in the country.
